toc highlighting doesn't work with globaltoc

As reported by Boris Kheyfets, the toc highlighting code doesn't handle globaltoc.html. Need to have js init code scan for those elements, will probably have to add an identifying marker to globaltoc output as well.

To upload designs, you'll need to enable LFS and have an admin enable hashed storage. More information
