5D/MII - Intervalometer "double shoots" in between sets

Sorry for the confusing title, not sure how to describe it better. Basically, for example, let's say I set an HDR sequence of 2 shots to be taken every 12 seconds. And let's just for this example say each exposure is about 1 second give or take.
What should happen based on my settings (and 99% of the time it works perfect) is this.
Second 1: (HDR Shot A) Shutter Open/Close - Exposure Writes to card Second 2: (HDR Shot B) Shutter Open/Close - Exposure Writes to card Second 3: Second 4: Second 5: Second 6: Second 7: Second 8: Second 9: Second 10: Second 11: Second 12: Second 13: (HDR Shot A) Shutter Open/Close - Exposure Writes to card Second 14: (HDR Shot B) Shutter Open/Close - Exposure Writes to card
The "bug" that's occurring is this:
Second 1: (HDR Shot A) Shutter Open/Close - Exposure Writes to card Second 2: (HDR Shot B) Shutter Open/Close - Exposure Writes to card Second 3: Second 4:(???) Shutter Open/Close - Exposure Doesn't Write to card Second 5:(???) Shutter Open/Close - Exposure Doesn't Write to card Second 6: Second 7: Second 8: Second 9: Second 10: Second 11: Second 12: Second 13: (HDR Shot A) Shutter Open/Close - Exposure Writes to card Second 14: (HDR Shot B) Shutter Open/Close - Exposure Writes to card
So the strange part is, every 12 seconds, a new HDR sequence should begin and like I said 99% of the time that is exactly what happens.
P.S. Many different settings, 10 second interval, 15 second interval, 3 shot HDR, 2 shot HDR, doesn't matter the error happens.
What's actually happening once in a while, and I'll be sitting there and notice it and GULP and be worried it's messing up my time lapse is almost immediately after the first sequence is taken, another 3 shots are taken. But what's weird is that later on upon review, the "DOUBLE TAKE" error/bug shots that shouldn't have been taken in the first place are in fact not on the card and are not exposed? So, yes this is a bug, no it's not messing up my time lapses (that I'm aware of), but, it is causing the mirror/shutter to take 2,3, however many actuations when it shouldn't be. Let me ask, could it be that I did not format the card (in camera) before use? I notice that no matter what settings both exposure settings and intervalometer settings, this error occurs. I'm on a Canon 5d/Mark 2.
