Provide extension version number to Sphinx

Bitbucket Importer requested to merge bitbucket/merged-pr-19 into branch/default

Was already merged in Bitbucket before import, marked as merged by the import user

In particular, the version number will show up in the Sphinx log. So instead of:

# Sphinx version: 1.4
[...]
# Loaded extensions:
#   sphinx.ext.autodoc (1.4) from s:\documents\code\colourettu\env-tests\lib\site-packages\sphinx\ext\autodoc.py
#   cloud_sptheme.ext.index_styling (unknown version) from s:\documents\code\colourettu\env-tests\lib\site-packages\cloud_sptheme\ext\index_styling.py
[...]

you get:

[...]
# Loaded extensions:
#   sphinx.ext.autodoc (1.4) from s:\documents\code\colourettu\env-tests\lib\site-packages\sphinx\ext\autodoc.py
