In the underlying data, the copies information and the removal information are
interleaved. And in the consumer code, the consumption could be interleaved too.
So make the processing closer to the underlying data, we fuse the two iterators
into one. Later, we will directly consume the underlying data and the iterator
fusing will be unnecessary.

Basic install::

$ make # see install targets
$ make install # do a system-wide install
$ hg debuginstall # sanity-check setup
$ hg # see help

Running without installing::

$ make local # build for inplace usage
$ ./hg --version # should show the latest version
